Summer Camp Cancellations

This form is for those enrolled in Summer Camps at Fitness Alive. Complete the form to cancel a date for your camp. You will get a confirmation email when your lesson is canceled.

Here is Fitness Alive’s Day-Off Camp cancellation policy:

If you need cancel your child's summer camp enrollment, you must do so in writing below. You will not be charged any future payments owed; however, refunds will not be given for any payments , such as deposits, already made.

** The balance payments for 2026 camps will run on June 2, 2026. Therefore, if you need to withdraw your child from camp, you should do so before the balance pay date of June 2nd so that you are not charged. **
